We welcome and encourage healthcare providers to support their patients in exploring participation in our clinical studies and to remain involved throughout the process. It is essential that patients remain directly informed—we must be able to communicate with both the provider and the patient to ensure transparency and continuity of care.

Follow-Up Care and Collaboration

Post-treatment follow-up is critical. We require exams at 1, 3, 6, and 12 months, and we strongly encourage your participation in these follow-ups to help monitor and document patient outcomes effectively.

How to Refer a Patient

We recommend reviewing the Clinical Studies section of our website and then discussing possible participation with your patient(s). If they are interested, please visit the Contact Us page and complete the form, including:

  • Your full contact information
  • The patient’s full name and date of birth
  • The patient’s email and cell phone number
  • A brief medical history and description of the problem
  • An indication of which study appears most applicable

What to Expect After Submission

We will respond via email to provide information about the appropriate study. If the patient’s condition is not a match for our current research, we will offer guidance that may assist you and your patient in finding suitable care options.

Submitting Medical Records

Ultimately, we will require a copy of the patient’s most recent examination—whether ophthalmic, neurologic, or general medical. While our email system is HIPAA-compliant, we recommend faxing medical records to (833) 661-1139 for the highest level of data security.
